Below is a general job description (JD) for a Patient Care Coordinator:
Job Title:
Patient Care Coordinator In this role, you will have the opportunity to provide administrative support to patients, fellow employees and referral sources through the coordination and administration of the front office activities.
The ideal candidate will have a background in an administrative role with strong computer proficiency, and customer service.
Your Impact:
Responsible for patient appointment scheduling.
Obtaining insurance authorization.
Creation of new patient charts and maintenance of patient records in the EMR.
Scanning and correctly filing patient documents in the EMR and ensuring complete and accurate information Managing incoming phone calls.
Providing financial counseling for patients and posting over the counter (OTC) payments.
Daily document research on an electronic health/medical record system.
Minimum
Qualifications:
High school diploma or GED required.
Requires less than 1 year of customer service, administration or related experience.
Must have, or be eligible to obtain, a valid driver's license.
Must be able to learn our EMR system within 90 days.
Be able to provide exceptional customer service.
Knowledge and Skills:
Strong interpersonal, oral (including telephone) and written communication skills, including the ability to follow written and verbal directions.
While performing this client facing position, working with a diverse population in multicultural settings, there will large amounts of communication (oral and written) including but not limited to clinicians, patients, physicians, physician's office representatives, other staff members (field associates, managers, directors) and/or business representatives.
Ability to work with physically challenged individuals.
Resourceful and flexible team player who excels at building trusting relationships with patients, referral sources and colleagues.
Proficient computer skills including Windows based office technologies (ex.
Word, Excel, PowerPoint), e-mail and automated billing systems.
Ability to use all necessary office equipment, facsimile machines, calculator, postage machine, copiers, etc.
Excellent organizational skills and the ability to manage multiple tasks (ex.
multitasking) with a high degree of detail orientation while working in a fast-paced environment.
Proficiency with basic math and accounting skills.
Ability to key 40 words per minute with accuracy preferred.

Possess excellent customer service and communication skills.

Patient Communication:
Act as a primary point of contact for patients and their families, addressing their inquiries, scheduling appointments, and providing necessary information.
Explain treatment, insurance, medical notes, prescriptions, and billing procedures ensuring they understand their document requirements.
Care Coordination:
Collaborate with healthcare professionals, including physicians, nurses, and allied health staff, to ensure the proper coordination of patient care.
Monitor and update patient records to reflect their current information as received.
Appointment Scheduling:
Schedule and reschedule patient appointments as needed, considering the availability of healthcare providers and the urgency of the patient's condition.
Follow up with patients to remind them of upcoming appointments and document requirements.
Insurance Verification:
Verify patients' insurance coverage and assist them with understanding their insurance benefits and out-of-pocket costs.
Process insurance claims and coordinate billing with the billing department.
Patient Advocacy:
Advocate for patients' needs and concerns within the healthcare system, ensuring they receive appropriate care and support.
Help patients access necessary resources and community services.
Documentation:
Maintain accurate and up-to-date patient records, documenting all interactions and care coordination activities in the EMR and in the correct file.
Team Collaboration:
Collaborate with other healthcare professionals to enhance patient care and improve administrative and care delivery processes.
Participate in care conferences and meetings to discuss patient progress and individualized care plans.
Participate in company meetings as needed.
Qualifications:
Associate's degree Strong communication and interpersonal skills.
Knowledge of medical terminology and healthcare procedures.
Familiarity with electronic health records (EHR) systems.
Ability to work in a fast-paced and sometimes stressful healthcare environment.
Empathy and compassion when dealing with patients and their families.
Knowledge of healthcare regulations and compliance.
